I think it's time for me to stand up in the bus next time and tell everyone that I have a well-developed hydrocele, and if someone asks what it is, I shall tell them that it's a fluid accumulation in the scrotum, so much so, that the scrotum swells up huge, becomes rock hard to the touch, but is not deadly, not catchy, it's bening. However, it looks weird, and it makes moving and sitting about awkward. Then I shall show them.

I dry my hair and dress reasonable and put on make up when I go to the shrink not because I am hiding anything but because if I show up in mismatched clothes, wet hair, and no make up, they assume I am depressed. True story.

I wouldn't be uncomfortable. I would see if they needed any help or anything though, just to make sure they were ok and not hurt or anything.
