I think you answered your own question. They are so upset or hurt that they DON'T think before saying what they say.
They probably do it because they feel like they are being attacked by the other person's words. Hmm, does that make sense or should I check in to a mental facility?

I caught myself doing that a couple time and I was like, what did I just do
 I think like most of us do. our emotions at time get the best of us. Just some know themselves better then others and like puffins said, some people just don't care

it's natural instinct to speak before you think. We all do it as kids. We train ourselfs to hold our emotions in and plan our words as we get older.

This is how it works. A very large number of people in the world, don't care about anyone or anything, but themselves. Thus they just say what they want to anyone. Much like Brian said, its stupidity. People lack the common sense to know when to do what is right and not to something that is wrong.
