Then leave it alone. You will alienate a friend. Plus, you'd need some proof.

tell her and then she has to decide. if he is, hopefully she'll come to her senses and run to a Dr!

Don't get involved. You'll be put in the middle of the situation in one way or another, and THAT will mean you will lose your friend. It's usually best for people to figure things out for themselves in some cases, and this would be one of those cases.
